Hospital Acquired Infections, also known as Nosocomial infection, is widespread in hospitals but not in a positive aspect because it is preventable. Infection concerns lead to various research and an ongoing study about evidence-based practice for the prevention and quality improvement of healthcare for Hospital-acquired infection prevention. According to the National library of medicine, nosocomial infections are more than double the mortality and morbidity risk for hospitalized patients, resulting in an estimated 20,000 deaths a year and an increase in hospitalization costs in addition to increased morbidity and mortality risk. Therefore, hospital-associated infections have been identified as one of the most crucial patient safety concerns in the healthcare setting. It all starts with proper handwashing, where we can avoid infections related to CLABSI, CAUTI, and hospital-acquired pneumonia. Cleanliness is the key to preventing any infection. Proper handwashing, an aseptic technique in taking care of both peripheral and central lines, and proper care of urinary or suprapubic catheters are essential in infection prevention. I believe that the best nursing theory is the mother of all nursing theories, “Florence Nightingale’s Environmental Theory,” concerning sanitation and cleanliness. This theory touched on points essential for infection prevention, including clean water and air, basic sanitation, cleanliness, and light.

Nightingale also believed that a clean and healthy environment was fundamental to healing. In the Hospital I worked, I am an active member of the Hospital-acquired pneumonia prevention committee, and according to evidence-based practice, oral care is vital for preventing pneumonia. Many patients have a self-care deficit, per Dorothea Orem’s theory of self-care deficit. These patients are dependent on caregivers for their ADLs, including oral hygiene. Patients can go on days without oral hygiene if not assisted in brushing their teeth. Thus, bacteria grow and eventually develop into pneumonia. Nurses should show compassion to these patients by reflecting on Watson’s theory of caring. Elderly patients are slow with their mobility; if you don’t have the care and compassion to be patient with them, you will lose patience. As part of our hospital response to these issues, healthcare providers must assist or remind patients to perform oral care at least twice a day or once a shift and chart in the electronic health record (EHR). Also, education is vital, and often, we watch each other, including doctors and other healthcare teams, “foam in and foam out” upon going to every patient’s room. Handwashing is simple, but some healthcare professionals often neglect sanitizing their hands. During the pandemic, other hospitals do not wear face shields, and some doctors argue that they don’t need to wear face shields anymore since they are wearing masks. We must ensure that everybody wears the right PPEs within the Hospital’s policy upon going to isolation rooms. Although various nursing theories can be applied in our daily nursing actions, the cleanliness of the environment is still a basic yet essential commodity in nursing and medicine. Nurses have a critical role to play in the provision of care that promotes safety, quality, and efficiency. In doing this, they practice in accordance with their worldview. According to Garrett (2018), worldview refers to the sets of assumptions and beliefs that influence the practice of nurses. It represents the manner in which nurses interpret as well as explain their experience. Culture has a significant influence on the worldview of nurses. It provides them with ontological and epistemic foundation that determines their belief system. Therefore, this essay examines my personal worldview, nursing theory that aligns with my philosophy, and an example in which the theory could help me in addressing a healthcare issue. It also examines the manner in which the worldview will assist me in developing my future practice as a nurse.

I believe that nursing practice is holistic in nature. It takes into consideration the multiple aspects that interplay to influence one’s response to nursing care. The aspects include religious, spiritual, and cultural elements that influence the uptake of nursing care. Patients have religious practices that must be respected during the provision of nursing care. It is important that nurses incorporate the diverse religious practices of their patients into their plan of care. This is critical because religious beliefs and practices can influence the uptake of a given approach to healthcare. For instance, it would be appropriate for the nurses to incorporate the services of clergies in situations where a patient believes that the health problem could be solved with a combination of medical and religious interventions (Mauk & Hobus, 2019).

Moreover, I also believe that culture has a significant impact on the provision of quality, safe, and efficient nursing care. Thus, the practice of nursing should take into consideration the cultural values, beliefs, and practices that influence care. Nurses should demonstrate cultural competence when working with patients from diverse cultural backgrounds. The competence is developed by being aware of the values and beliefs of the patients and respecting them. Nurses need to work in collaboration with their patients in identifying the cultural aspects that need to be considered in the provision of healthcare and their roles in promoting them (O’Brien, 2018). Through it, nurses will provide holistic care that is not only focused on the patient’s health problem but also the factors influencing the recovery process.

Nursing Theory

Theories have a significant influence on the care that nurses offer to their patients. They inform the decisions of care that the nurses embrace. Theories also form the basis of research in nursing. As a result, they also inform evidence-based practice in nursing. One of the nursing theories that inform my practice as a nurse is Virginia Henderson’s theory of need. The theory asserts that the focus of nursing care should be to enable patients to have the independence to undertake activities that will meet their health needs.

Henderson observed that optimum health could only be achieved with the provision of care that meets the fundamental needs of the patients. The assumptions of Henderson’s theory resonate significantly with my assumptions on nursing practice. According to her, nurses have the professional responsibility of providing patients with care until a period when they are able to take care of themselves. Nurses should also be devoted to their professional practice. They should explore the ways in which optimum nursing care is provided to match the healthcare needs of the patients. Henderson also asserts in her theory that nurses should be educated for them to provide appropriate care to their patients (Peate, 2019). They should undergo a long period of training to inform their safe, efficient, and quality practice in their settings.

My philosophy is centered on providing care that addresses the basic concepts of nursing care. The concepts include nursing, health, individual or patient, and the environment. These concepts are also evident in the theory of Virginia Henderson. According to her, nursing entails assisting patients in doing things they could have done if they were not sick or ill (Raingruber, 2017). Individuals are perceived to have fundamental needs that must be met. They may need assistance in situations where they lack the independence in meeting them. Henderson asserts that the environment entails both the internal and external factors that influence health. As a result, nurses provide patients with environments that promote optimum health and recovery from a disease. The last aspect in Henderson’s theory that influences my practice is the components of needs that nurses must promote in their patient care. Therefore, this theory reinforces my practice of nursing by acting as a guide on the manner in which prioritized care is given to those in need. Example of Current Practice

An example of current practice that can be resolved using my worldview and nursing theory is the need for 24-hour nursing care for patients in hospital settings. Generally, the provision of nursing care is a continuous process. Nurses engage in the provision of continuous care as a way of ensuring that actual and potential needs of patients are addressed in a timely manner. The need for this practice is informed by my worldview as well as Henderson’s theory. As shown earlier, nursing practice should be holistic in nature. It should address current and emerging health needs of the patients. The needs can only be identified with the provision of continuous care that aims at responding to immediate needs of the patient for optimum health and wellbeing. Henderson’s theory also informs the need for continuity of nursing care. According to her, patients have multiple healthcare needs that must be addressed by the nurses (Raingruber, 2017). Consequently, it make it relevant for nurses to engage in continuous care for them to provide care that meet the actual and potential needs of the patients.

How my Worldview and Nursing Theory will help in Developing my Future Practice

My worldview and nursing theory will help in developing my future practice in many ways. Firstly, it will help me engage in research that will contribute to the practice of safe and efficient care. It will also help me engage in care that protects the rights and needs of the patients. The principles of my worldview and the theory will also help me explore the ways in which care in our setting can be improved. This includes exploring the need for a change in institutional practices and policies to influence the quality of care offered to those in need (Raingruber, 2017). Therefore, I believe that my worldview and nursing theory will influence significantly the development of my future practice as a nurse.

In summary, my worldview about nursing has a significant influence on the care that I provide to the patients in need. A worldview denotes ways in which individuals translate the world around and develop a philosophy in their approach to care. A worldview comprises of the ideas and beliefs that influence one’s thinking about different concepts that include cultural, religious and spiritual aspects. Nurses must develop their worldviews for effective care delivery through interactions with patients from diverse backgrounds (DeNisco & Barker, 2017). Nurses are expected to offer care that meets cultural, spiritual, social and religious needs of patients. The purpose of this essay is to reflect on my current and future practice and the ways that worldview and nursing model will influence these aspects.

Personal Worldview

My personal worldview is shaped by my experience and interactions with a multidimensional culture of care that values respect for personal views of other individuals based on their experiences and perspectives. I believe that as nurses, irrespective of one’s specialty, we are called to serve, either through a religious understanding and perspective, or through one’s philosophical underpinnings. Nursing encompasses taking responsibility for continual growth in humanistic interactions, development of character and showing compassion to sick and suffering patients with different conditions. I believe that nurses are genuinely passionate about caring for patients and are called to serve. The implication is that as a nurse, one must develop spiritual love and caring as qualities to build therapeutic relationships with patients and improve care delivery. Spiritual care emanates from the heart and nurses should always focus on enhancing patient care through a holistic approach that include physical, mental, social and spiritual components (DeNisco & Barker, 2017). Respect for life is a core component of my cultural care worldview and I believe that God places nurses in unique caring positions to demonstrate his love for humanity. Spirituality is an essential component in many world cultures and by believing in a higher power, nurses do not consider themselves as the only responsible for better patient outcomes. Cultural sensitivity is a critical aspect of care delivery as it implies that nurses understand the differences that exist among patients and their families and how to treat each patient in a unique manner for better outcomes. It is also important to realize that these differences are sources of strength and can help define one’s nursing practice philosophy.

Theory Aligning with My Worldview and Its Reinforcement

As a registered nurse who wishes to specialize in informatics, I believe that diversity and universality are elements shaping the healthcare industry and essential aspects of attaining patient-centered care. Imperatively, the specific nursing model that aligns with my worldview is the Leininger’s theory of culture care diversity. Also called the transcultural nursing theory (TCN), the model’s concepts and propositions require nurse practitioners to use culturally respectful and congruent nursing care based on patient’s beliefs, values and cultural practice (McFarland & Wehbe-Alamah, 2019). The model requires nurses to use culture-specific nursing care that is not only holistic but also recognizes the patient’s values and their general worldview.

The similarities between the model and my worldview are evident. Both the model and my worldview consider respect for patient’s cultural beliefs and opinions. The theory and my worldview also appreciate the respecting, accepting and integrating patient’s cultural aspects and practices into care plans does not affect the quality of care offered. The implication is that the integration enriches care quality by making it culturally significant and sensitive to the needs of the patient and their family. My personal worldview focuses on empowering patients through acceptance and appreciation of their cultural preferences (Sagar & Sagar, 2018).  Therefore, Leininger’s culture care theory reinforces this by emphasizing a patient advocacy philosophy and a patient-centered approach to nursing care. The implication is that both the theory and personal worldview emphasize the need to uphold the bioethical principle of independence or autonomy.

The philosophy entails understanding and appreciating the uniqueness of each patient’s cultural beliefs and values on issues like diet, traditional medical interventions, healing through faith, and the place of prayers among others. At the core of these similarities is the advocacy of culturally congruent nursing care for each patient at individual levels with no prejudice or biased judgment and stereotypes (McFarland & Wehbe-Alamah, 2019). The theory reinforces my perspective as it implies that culturally competent and acceptable nursing care does not require one to change their worldviews. Contrary, it implies that nurses must appreciate, accept and embrace each patient and their uniqueness and device evidence-based practice strategies to develop appropriate care plans for optimal outcomes.

Example in the Current Practice

It is not always easy and certain to implement what the conceptual propositions of theories advance in clinical settings. However, in situations where there is convergence between personal worldview and the nursing theory’s suggestions, the incorporation of such evidence becomes easy and effective. The two aspects become essential in changing a current or past practice that may not align with the ideology propagated by both worldview and theory. The present practice of rejecting all types of traditional or alternative and complementary medical interventions because of lack of evidence on their effectiveness based on empirical research is a practice that rejects the incorporation of patient’s opinions and views (Wilson et al., 2018). Nursing like other medical and healthcare disciplines only recognize the use of therapies and interventions supported by research because of the concept of evidence-based practice. It implies that for an intervention to be deployed in managing a patient’s condition, it must be based on strong empirical evidence that supports its effectiveness.

Contrary, most of the culturally accepted remedies and alternative medical interventions are not backed by empirical evidence for their efficacy. The issue is that rejecting these interventions even in instances where the patient believes in their working and effectiveness negates the concept of culturally component and sensitive care. Therefore, my worldview and the transcultural care model would help me to correct this problem by prescribing acceptance and education for practitioners and assisting the patient understand the basis of the present position. As a nurse, one must no judge and dismiss a patient’s preferences as inconsequently and with not effective outcomes (Wilson et al., 2018). Instead, the nurse should accept the preferences and use tactful ways in a culturally competent manner to educate the patient on the facts concerning the use of both alternative medicines and conventional approaches.

Influence on Future Practice

Nurses must recognize that their personal worldviews should not always be congruent to patients’ because of different cultural and social reasons. Imperatively, the nurse must be culturally competent and provide culturally congruent care by adapting their views and perceptions to a patient’s worldview. Therefore, having a multidimensional culture of care approach and getting support from Leininger’s model will be critical in my future nursing role and practice (McFarland & Wehbe-Alamah, 2019). As a future nurse informaticist, I should incorporate cultural dimensions in my role for effective interpretation of data and making effective decisions that will enhance care delivery. As an informatics nurse, integrating cultural competence in all aspects of data analysis will help in effective decision making and developing reports for easy implementation in practice for other providers. Through this approach, I will enrich my future career as an informatics nurse by ensuring that it is culturally sensitive and congruent to patient’s needs.

Conclusion

A nursing personal worldview is essential in helping nurses form effective interactions with their patients and provide culturally-aligned nursing care. As illustrated by the nursing theory and worldview, nurses must ensure that they embrace cultural practices that support patient’s preferences as such can enhance quality care and patient experiences.
