The apprehensions regarding patient safety and quality improvement in health care setting are mounting (Hurston, J. 2014). For decades, the remarkable utilization of technology in the health care setting, specifically the use of electronic health record (EHR) greatly contributes to the changes in the health care system. Technology for information conduction and dissemination has been advancing swiftly and steadily for more than a century.

Nowadays in the age of advanced technology, the electronic health record is now greatly recognize by anyone who uses it. For instance, a simple scenario at the hospital setting, wherein a patient is admitted from Emergency department, wherein admission process is initiated and entered into the computer by the registration staff. Then the order system is entered by the attending physician as part of the electronic health record. A name band is placed in the patient’s wrist that contains barcode which is then later scanned by staff whenever the patient receives treatment, medication, or procedure. Therefore, with this process of scanning the patient’s barcode, the right information entered during the registration will then be reflected in the computer. The patient then confirmed by saying the right name, birthday, and even allergy to any medication when asked by staff before any treatment begin. This process shows patient safety is achieved. In other words, when a right name indicated in the electronic health record as confirmed by the patient, errors can be prevented. Thus, quality of care is improved.

Using electronic health record (EHR) as an instrument can advance outcomes of care, decrease clinical errors, and operate as a cost saving clinical system (Hurston, J. 2014). EHR are intended to accumulate different and many kinds of data, such as patient’s demographic, laboratory and radiology results, placing patient’s orders, and sending messages to anyone within the system (Alexander, S., Frith, K.H., & Hoy, H., 2019) This set up is readily available through electronic health record.

Meanwhile, many of the operating machines that are utilized while giving care to the patients have an implanted clinical decision – support systems (CDSSs) like a pathway in which it in provide an alarm to the attending physician and assigned nurses whenever certain parameters are beyond the actual normal limits. As a whole CDSSs has the ability to provide recommendation when certain action is needed and an alarm when certain action is not accurate, this alone, lead to  patient safety and achieve improvement in the quality of care. Considering that patient satisfaction is the most important goal as the technology is consolidated into practice (Hessels, A., Flynn, L., Cimiotti, J. P., Bakken, S., & Gershon, R., 2015).

Different clinical scenarios can contribute to missing and possibly lack of data / information by any staff who accessed the EHR. For instance, busyness, complicated work surroundings, and inadequate workflow contribute as any staff haste in entering any data to electronic health record (Alexander, S., Frith, K.H., & Hoy, H., 2019).This then leads to errors that can potentially produce harm to any patient. Despite the advantages linked with EHR systems, challenges while using still continue to occur in healthcare setting. There are few existing challenges considered disadvantages in utilizing the EHR namely, the absence of interoperability, which is probably the main concern when using it, purchasing it with a very high price and not to forget how to keep it up to date and intact.

Anyone who uses EHR regardless which worksite one belongs, acknowledging the concerns, usefulness, and challenges that the system bring, we must not put our patients in danger due to errors that can be avoided. Hence, focusing our goal to achieve patient safety daily and keep the quality of care improved is all that matters.

Text slides are not meant to be read by the speaker, but by the audience. Lettering should generally be limited to four lines and should never be more than seven, including the title.

especially in clinical practice. Roles such as to reduce costs by focusing on primary care, health
promotion, and disease prevention as expert clinicians, faculty in schools, health care consultants,
entrepreneurs and scholars caring for multiple patient populations in various geographic areas and as
global health leaders (Zaccagnini, & Pechacek, 2019). As an expert clinician and academic scholar, the
DNP prepared nurse is able to improve patient care through initiation of quality improvement (QI)
initiatives, assisting with healthcare policy development. And also has knowledge and skills in healthcare
economics, finances, leadership, and quality improvement (Zaccagnini, & Pechacek, 2019).
Workflow is a repeatable series of standard tasks that needs to be completed to achieve specific goals
such as clinical processes in nursing. So, healthcare providers (HCPs) should be involved in the creating,
planning and the implementation of health information technologies (health IT), to improve support.
When an organization adopts an electronic health record (EHR) it changes the usual workflow standard.
Therefore, the workflow is redesigned to suit the new system. The workflow redesign is a process of
looking at all the aspects of the organization and mapping out current workflows and analyzing how the
work gets done in the organization and they plan for the future and create new workflows to increase
the organizations efficiency and the quality of health care. Workflow redesign helps most organizations
to improve their EHR implementation, their efficiencies, enhance health care quality and safety, remove
chaos from your current workflow, improve care coordination (HealtIT.gov., 2019).
As a DNP leader, I would assess to understand what will be involved, such as patients, a new EHR
system. I would communicate with the senior information technology (IT) analyst team to understand
what kind of system they plan to implement and then plan for a meeting with the nurses that will be
working with the EHR to inform them of the system and when training would begin. The training would
take place for 1-2 weeks at the most one month to make sure that all the staff is trained appropriately
and efficiently by IT professionals and the clinical specialist to integrate the EHR into their work flow.

During the implementation, the patients will be informed of the new EHR system and that the IT
professionals and the clinical specialist would be available to help the staff to trouble shoot any
problems encountered while working with the patients (Fecher, McCarthy, Porreca, & Yaraghi, 2020).
The term “Magnet” is a gold standard term for nursing practice in hospitals which is used to delineate
those who are able to attract and retain qualified nurses based on five goals such as transformational
leader, structural empowerment, new knowledge, innovation, and empirical outcomes and
development. The effectiveness of magnet hospitals is the existence of work environments that focus on
decentralized decision-making, autonomy, control over practice, resource adequacy, supportive
management, effective inter-professional communication, and career development.
As a DNP leader in a magnet hospital, I will ensure that the staff are well trained to maintain our nursing
excellence standards and the structures needed for the training are in place during training as well as
during implementation and maintain communication with the staff in terms of when this will take place
so that they are well prepared. Adequate structures such as adequate computers, the IT professional
team, the scheduling of their time for the training is adequate with enough staffing to cover patient care
for those in training to maintain patient safety and excellent nursing care. Providing adequate
knowledge of the improvement in the quality of nursing care that this new system will provide will
enhance their desire to learn and implement the program.
In a rural setting, the pace would be slower, so, as a DNP leader, I would have to reevaluate my strategy.
I would have to also give them time to assimilate the need for change to a new EHR system. They will
need more time to train and for implementation knowing that the patient influx maybe slower and the
staffing may be less adequate. As the DNP leader, I will collaborate with all the leadership teams to
make them aware of the process change taking place and the need for their collaboration with my team,
explain to the patients the new changes that will take place. The leader has to make sure that all
structural resources are in place as well.
