Introduction

qJean Orlando developed deliberative nursing process theory in 1961

The theory of focus in this presentation is Ida Jean Orlando’s deliberative nursing process theory, which she developed in 1961. The theory recognized the crucial role that nurse-patient relationship plays in ensuring the realization of optimum care needs for diverse populations. Ida stressed that nurses should strive to establish therapeutic relationships with their patients with the aim of providing care that responds to their prioritized and potential needs. Ida also considered nursing profession an organizing role that is centered on the diverse patient needs. Jean incorporated the aspects of nursing process such as assessment, diagnosis, planning, implementation, and evaluation in her model. The use of the phases of the nursing process ensures that nurses develop care plans that are appropriate to the needs of their patients (Gonzalo, 2021).

Nurse Ann’s Initial Action

Nurse Ann has several functions to play during her initial assessment of the family. One of them is developing hypotheses. Hypotheses depend largely on the subjective assessments the nurse undertakes during the visit. Some of them include Sam’s forgetfulness and impaired vision that places him at a risk of injuries and self-care deficits, Susan’s anxiety and caregiver fatigue in meeting her father’s needs, and Sam’s feelings of overburdening her daughter. The other action that Ann should undertake is collecting objective and subjective data from Susan and Sam. The information is important for use in validating the hypotheses developed from the subjective assessment of the family (Toney-Butler & Thayer, 2022).

Questions for Susan

qHow can you rate your father’s ability to care for himself and meet his daily needs?

qWhat types of activities do you assist your father in undertaking them?

qHow can you rate the kind of stress you are experiencing from caring your father?

qWhat coping mechanisms or support do you utilize to overcome these challenges?

The case study demonstrates that Susan is the only child responsible for caring her father. As a result, questions about her perception of her father’s ability to care for himself and meet his daily needs should be asked. The question will facilitate an understanding of Sam’s ability to meet most of his daily needs independently. Information about the activities that Susan undertakes while caring for her father is also important to guide in determining appropriate services for him. Information about Susan’s experience with caring his father such as stress level and coping mechanisms should also be obtained to determine the mental health impact of her father’s needs on her health (Toney-Butler & Thayer, 2022).

Nurse Ann’s discussion with Sam should focus on Susan’s concerns and his feelings towards them. The nurse should inform Sam about the challenges that Susan is experiencing caring for him. He should also encourage Sam to express his concerns, feelings, and needs. The nurse should facilitate a constructive discussion with Sam on the ways in which he can meet his needs of the daily living. The nurse should also obtain information about any activities of the daily living that have become difficult for him to undertake. The nurse should also brainstorm about the different social groups and support programs that Sam can use to achieve his needs. The information obtained from Sam will guide and validate Susan’s concerns and the developed hypotheses (Gonzalo, 2021).

Case study

Ann, a community nurse, made an afternoon home visit with Susan and her father. After the death of her mother, Susan had growing concerns about her father living alone. “I worry about my father all the time. He is becoming more forgetful and he has trouble seeing. Mom used to take care of him. I am not sleeping and I am irritable around him. Yesterday I shouted at him because he wouldn’t let me help him with his laundry. I felt terrible! I am at my wits’ end! My brothers and sisters do not want to put dad in a nursing home but they are not willing to help out. As usual, they have left me with all the responsibility. I work part time and have two small children to care for.” Susan’s father, Sam, sat quietly with tears filling his eyes. He was well nourished and well-groomed but would not make eye contact. Nurse Ann noticed that the house was clean and orderly. A tray in front of the TV had the remains of a ham sandwich and glass of ice tea. Mail was piled up, unopened on a small table near the front door. There was only one car in the driveway and the yard was in need of attention.

What questions does Orlando’s theory guide the nurse to consider in caring for Susan and Sam?
