The four spheres of political action in nursing are formed around the community, the government, professional organizations, and the workplace. These spheres point out ways that politics plays a role in each of these environments. The community sphere identifies how nurses can become more involved in their communities and within community organizations. This sphere also focuses on how policy influences community health. The community is a good place for nurses to gain support for health policy changes (Mason et al, 2016). The government sphere discusses how polices enacted at the local, state, and federal levels impact the nursing industry and its guidelines. The government component also explores nurses as political activists and how they can influence policy making on each level of government. In the professional organizations sphere, nurses are shown how being associated with professional nursing organizations can further nursing policy aims. This area discusses how being part of a larger organization can work in a nurse’s favor regarding shaping policy for professional standards, licensing, and acceptable credentials (Mason et al, 2016). The workplace sphere is a review of how policy influences the environments nurses work in. The workplace sphere talks about internal and external polices that affect the workplace and influence how nurses do their jobs. Each environment a nurse works in is a political arena (Mason et al, 2016).

According to Gillette (2017), hospitals have a big economic affect on the community and tax payer dollars. Government and private health insurance plans limit the number of days a patient can stay hospitalized in an acute care facility (Gillette, 2017). Therefore, policy is being revamped to make a push to send patients who require longer hospital stays to a “hospital within a hospital;” this is a long term acute care facility that is managed by an outside company (Gillette, 2017). My focus is on long term acute care because many older patients are affected by internal and external policies. Ethical issues that are raised center around the constraints that are imposed by public policy and who funds the long term acute care services older patients receive. The debate over who is eligible for funding concerns me because I question whether the legislatures making these decisions consider guidelines suggested by social and ethical theories.

I appreciated your definition of each of the four spheres and described their specific roles. In my opinion nurses are able to make policy and practice changes in many different aspects of healthcare, including community healthcare as you mentioned. Nurses have to ability to change issues directly correlated with the nursing profession, such as staffing needs, patient to nurse ratios policies, nursing education, etc. We are more than “just nurses”, we are policy makers, educators, entrepreneurs and our voice makes an impact. I agree that most nurses are unaware of the political aspect of our profession, including the need to be aware of laws impacting our practice and our patients. The more knowledge we gain from governmental laws, the bigger difference we can make in our workplace, the organizations we are affiliated with and ultimately the community we work in. This brings us back to the four spheres and the influence each sphere has on the nursing profession. Being aware of the value of each aspect and using that knowledge make positive developments can transform healthcare and patient care.

Hi Judith.  I connected with your post because I also work as a nurse in the Emergency Room and wait times have become a big problem for us over the past 5 or so years.  The first 5-7 years I worked in the ER patients rarely had to wait more than 15-20 minutes.  Since that time we have remodeled and expanded the amount of beds and many times we have wait times of 2-6 hours.  You mentioned boarding patients and how this puts the patients in the waiting room at risk.  I absolutely agree.  I started my career on the floor and when our beds were full that was it, no more patients.  In the ER the patients keep coming and every time we have to wait for a bed to open up for an admit hold we are delaying care of others who are sitting in the waiting room undiagnosed.  One of our triage nurses sits at the front desk and registers patients and keeps track of people in the waiting room.  That assignment can be the worst because half of your day is spent deflecting mean comments from people waiting.  Many times we send ambulance patients to wait in the waiting room if they have a lower acuity complaint and are stable.  In our ER we have advanced techs who work in triage with the nurses and if wait times are over an hour we order initial blood work, urine, and x-rays if necessary just to expedite the visit.  This does help because many times patients have labs resulted by the time the Dr sees them and then further diagnostic testing can be decided on.  So in addition to a full unit we are also medicating and getting labs and x-rays on waiting room patients.  It’s difficult and dangerous.  There have been policy changes over the years that attempt to open overflow units and move those ER patients who are waiting for beds over so we can make room for ambulances and waiting room patients.  It’s really difficult when the census is high because everyone in the hospital feels it, but it’s important for policies within hospitals to have a plan to move boarded patients out quickly so those waiting can be evaluated.
