This project proposed the adoption of mindfulness meditation to address the mental health conditions among seniors admitted in a managed long-term care setting. Mindfulness mediation is expected to help patients to remain mindful all through the day, especially during difficult situations by reducing racing thoughts, relieving negativity, and calming the body and mind. This intervention is anticipated to lower stress and stress reactivity in seniors with mental health issues, leading to increased quality of life and better outcomes.

Clinical Problem Statement

            Mental health conditions are major community health concerns that are associated with negative impacts such as poor quality of life, compromised activities of daily living, reduce satisfaction with life, and ruin personal relationships (Biering, 2019). Moreover, mental health conditions are attributed to the highest suicide rates among seniors. Mental illnesses in elder persons also affect their families by creating tensions, stress, uncertainty, and major shifts in how families live their lives. Therefore, it is essential to address this health concern in nursing practice to enhance the quality of life and better outcomes among seniors with mental health issues.

Purpose of the Change Proposal

            The proposed nursing issue in this change proposal is the management of mental health and psychological conditions of elderly patients or seniors living in a managed long-term care setting. Today, mental health issues are increasingly becoming common among elderly patients and are associated with dire short-term and long-term effects on health such as poor quality of life, compromised activities of daily living, reduce satisfaction with life, and ruin personal relationships (Biering, 2019). Mental health conditions are also attributed to the highest suicide rates among seniors and also affect their families by creating tensions, stress, uncertainty, and major shifts in how families live their lives.

            Therefore, mental health conditions should be regarded as serious health concerns among the elderly and should be addressed in nursing practice to enhance the quality of life and better outcomes among patients. Consequently, the purpose of this change proposal is to advance the adoption of mindfulness meditation as an intervention to address the mental health conditions among the seniors admitted in a managed long-term care setting. Students will synthesize the components developed in prior topics to formulate a written evidence-based account of the capstone project. Additionally, the student will integrate the following into the written evidence- based account: a plan for evaluating the nursing intervention, a change or nursing theory, and a discussion on how potential barriers will be overcome.
Objectives:
1. Produce a written comprehensive evidence-based capstone project change proposal using components
developed in prior topics of the course.
2. Integrate a plan for evaluating the nursing intervention in the written comprehensive evidence-based
capstone project change proposal.
3. Integrate a change or nursing theory into the written comprehensive evidence-based capstone project
change proposal.
4. Identify potential barriers to the implementation of the change proposal.
5. Integrate how the identifed potential barriers will be overcome.
6. Integrate refective practice into the practicum refective journal.
7. Demonstrate interprofessional collaboration during the creation of the capstone project change proposal.
