that had value in the past need to be re-evaluated as we move towards
healthcare reform, integrate evidence-based practice and/or best practices and
work towards achieving positive patient outcomes and educate nurses who will be
prepared to practice in this ever changing
health care environment. Recall a situation when you were asked to
perform a specific intervention and/or practice idea based on authority or
tradition. Describe the the intervention or practice. Based on your knowledge
of evidence-based practice, does this practice still hold value now? Use an Evidence-Based Guideline or nursing
specialty organization Standard of Practice to support your answer.

Evidence based practice (EBP) is the use of assorted sources of knowledge that are backed by science and research (Horntvedt et al., 2018). Patient preferences, the learned experiences of staff, and published works of research can all contribute to EBP. The only information that should be included in EBP must first be deemed to be reliable evidence (Horntvedt et al., 2018).

Nursing practice problems are problems that the nurses have the control to change. They do not need to have the doctor create the order. Nursing problems need to fall within the nurse’s independent scope of practice. An example of a nursing practice problem would be interventions related to pressure injury prevention. Staff assessed the effectiveness of pillows vs. wedge usage to see which intervention has the highest effectiveness in preventing pressure injuries (Powers et al., 2020). In this case they noted that none of the patients had a pressure injury, however, the angle of the turn was well maintained with the wedge positioning after one hour (Powers et al., 2020).

Medical practice problems relate to things that are in the provider’s scope of practice. This includes things like medication orders and lab draw orders. Those things rely upon the external factor of a doctor and their willingness to order a new intervention. Therefore, they are not appropriate to focus on for a PICOT question nor EBP.

PICOT questions need to be based on nursing problems and not medical practice so that they can be implemented by nursing staff. These are things that nurses should be able to have control over and change independent of a doctor’s order.

DQ 2 According to the American Association of Colleges of
Nursing the Masters prepared nurse ” Recognizes that the master’s-prepared
nurse applies research outcomes within the practice setting, resolves practice
problems, works as a change agent, and disseminates results.” Many
research ideas emerge from practice and/or the educational environment.
Formulating a well-built research question is challenging. Asking the right
question facilitates the search process when conducting a review of the
literature. Read the article “Asking the clinical question: A key step in
evidence-based practice found in the supplemental activities folder and review
the slide presentation included in this unit. After reading the article and reviewing
the slide presentation use the PICOT framework and practice creating a PICOT
question. Create a PICOT question for a
quantitative study and a PICOT question for a qualitative study that relate to
your area of interest. Obtain an Evidence-Based Guideline (EBG), Standard of
Practice, or Best practice in nursing education that pertains to your
question. In responding to your
colleagues’ postings offer alternative questions and rationales for your
suggestions.
