In R & S Ch 1, you read about the Spiral Path of Nursing Knowledge, What do you think about the practice of nursing being central in developing—not just applying—nursing knowledge? Do you develop new ideas from your practice experiences?

While I was reading about the Spiral Path of Nursing Knowledge, I felt myself relating to the information I was reading. According to Reed & Shearer (2018), the path of nursing is considered ongoing and nonlinear. The path can bring unpredictable changes every day. I feel that this statement describes nursing perfectly. Nurses never know what their shift will bring. I recently graduated from nursing school and have developed my own clinical experiences. Every day truly is unpredictable. I never know what my shift will bring, and I end up coming home with more knowledge than the next day.

Further in the reading, Reed & Shearer (2018) discuss how nurses build knowledge by testing theories against observations in practice and elsewhere. With continuing research being performed, many interventions nurses do provide care for their patients could be different in the upcoming months. During the pandemic, nurses have had to adapt to care for their patients via telemedicine. They had to learn how to assess their patients without them being right in front of them. Also, learn how to use the technology. Through every unpredictable change, nurses have worked hard to continue to shine in their profession.

I believe that the practice of nursing in being central to the development of nursing knowledge. Reed & Shearer (2018), state that knowledge development is above all influenced by each turn. Nursing has changed throughout history. The information nurses learn in nursing school could be different from what they practice today. Therefore, nurses are required to have contact hours to renew their licenses. I have not had to renew my license yet but taking a graduate-level class counts towards your contact hours. Since nurses are always learning, I wanted to pursue my degree to gain more information that can help me treat my patients.

Throughout my personal practice experiences, I have developed new ideas. Nursing is a profession that you gain experience over time. Each patient of mine teaches me something that a textbook couldn’t. I have learned that some patients do not fit the textbook definition of a disease. I have had to learn how to trust my gut. I am still learning how to trust my gut and be confident in my assessments. I am still a new Registered Nurse, but within the past couple of months, I have grown and developed new ideas. I am still learning each day, but my nursing experience has been influenced by my past experiences.

After reading chapter 1 of Nursing Knowledge and Theory Innovation, it is evident that the practice of nursing is the driving factor of theory development and nursing knowledge.  It is also clear that new ideas and theories are developed as a result of personal nursing practice experiences.  Reed and Shearer state that, “The theories of today need to be sensitive to the given situation and changing problems and experiences, yet also illuminate patterns that broaden understanding of nursing phenomena (Reed & Shearer, 2018, p. 3).”  The nature and practice of nursing is ever evolving, therefore, nursing education and knowledge must also reflect the changes driven from practice.

 

Reed and Shearer describe the Spiral Path as a way to relate nursing experience and knowledge to nursing theories and state that knowledge in a “fundamentally continuous process” (Reed & Shearer, 2018, p. 4).”  This knowledge is gained upon every “turn” of the spiral path, this knowledge and learning process is always being expanded upon and developed.  By doing so, nursing practice is changed and improved upon.

 

The philosophical, empirical and theoretical dimensions are gained through nursing practice.  Philosophical dimensions provide perspective on morality (Reed & Shearer, 2018).   Empirical dimensions are learned through the senses (Reed & Shearer, 2018).  Theoretical dimensions link empirical and philosophical dimensions through development of theories based upon nursing practice and experiences (Reed & Shearer, 2018).  “Theories are open systems, which are amendable to change and possible improvement by critique, modification, and refinement through ongoing theory development and evaluation (Reed & Shearer, 2018, p. 6).”

 

The central theme that I noted is that nursing knowledge is continuous and constantly changing based on current trends.  Theories guide our practice as nurses, however they must be fluid enough to be able to develop and change as knowledge and practice changes.  New ideas will always be derived from experiences in practice.

Nursing practice is essential to the process involving knowledge development within healthcare. The spiral path described by Reed & Shearer (2018) demonstrates the nonlinear process by which nursing knowledge is developed, influenced by and interrelated to various components. Theory may appear as central to nursing knowledge in a visual sense, however I understand theory to be more accurately described as a tool.  Nurses utilize nursing theory as an instrument for implementation of new ideas originating from their own personal perspectives, skills and experiences in practice. Theories guide advances in understanding within the healthcare realm and are impacted by nursing interpretations and implementation of inquiries, attesting to nursing practice as fundamental in creation of new nursing knowledge. Our text discusses nurses’ ability to interpret anything by which has been obtained through the senses as launching points for the development of new knowledge (Reed & Shearer, 2018). Patient assessment involves observing the spectrum of wellness and illness using our senses, feeding the natural process of knowledge generation. Every nursing interaction provides potential for knowledge development.
