Some subjective findings that would indicate the need for further assessment would be despair, anhedonia, a decrease in appetite, insomnia, overwhelming sadness, fatigue, and inability to concentrate. Some objective findings would be anxiety, muscle tension, inability to sit still, unkempt appearance. I would use a personal history assessment, Depression Questionnaire, and SAD PERSONS Suicide Risk Assessment (Weber, 2018, p 82). It is important to check for substance abuse problems as people often self-medicate, I would ask about alcohol and drug abuse using the AUDIT (Weber, 2018 p 76).

I live in a smaller rural community but there are resources in the area for help with depression. Helping Hand Behavioral Health is located in the neighboring town. They have a variety of programs and both in and outpatient services to help with mental illness and depression. They have evaluation and counseling services, group therapy, living skills, nutrition, and also provide transportation. They also have pet therapy and exercise programs. I am happy to see that they have a very well-rounded program that not only offers mental health services but promotes whole-body wellness.

I had it in my head that yesterday was Sunday, and suddenly realized that it was today! Therefore, I will be explaining the Ketogenic Diet as well. I had to research this one a bit, as it wasn’t covered in our textbook. I’m familiar with the Atkin’s diet and South Beach diet for weight loss, but they have been controversial. Generally speaking, a Ketogenic diet is low in carbohydrates and high in fat. More specifically, carbohydrates lower than 50 grams and fat greater than 70%. I was interested to learn that it has also been used to reduce the frequency of seizures in children with “…refractory epilepsy, patients in whom resective epilepsy surgery is not feasible.” (Lambrechts et al., 2016, Introduction section) This diet is also being studied in use for patients with insulin resistance, cancer, and a number of other health issues. Part of the controversy to the diet focuses on the amount of saturated fat that can be consumed while still following the diet. However, some supporters of the diet have noted a decrease in LDL cholesterol. Also, it is generally assumed that it requires one to eat a high amount of animal products, but it can be adapted to a vegetarian or vegan diet as well. (Ludwig, 2019)
Questions I would ask my cousin:
*What do you hope to achieve with this diet? Weight loss? Blood sugar control?
*Have you consulted your doctor? Will you follow up?
*What kind of diet do you currently follow?
*Will this be temporary, or a life change?
*Are you planning to do this on your own, or will household family members also be following?
*Are you aware that it is an expensive diet?
*Do you plan to include exercise if your aim is to lose weight?
*Do you have any pre-existing health conditions? Diabetes? CVD?
Advice I would offer would be to consult his/her primary care doctor for guidance. I would also suggest that they read up on potential side effects of a drastic change in diet. Some have noted gi upset with a change of fat intake. (Lambrechts et al., 2016) I would recommend that my cousin stay hydrated and make sure that the carbohydrates they are ingesting are complex vs simple to maximize the amount of energy stored. (Janet R. Weber Rn Edd & Kelley, 2018) I would encourage him/her to do their research, and make sure that it is a diet they can follow. I would advise them that it is not a diet that one can have “cheat days”.
